United Home Care serves patients in recovery after discharge from the hospital and/or nursing home. After physician’s orders are issued, our nurses are dispatched to your home to perform an assessment and, if appropriate, an admission. A plan of care is then designed specifically for your needs and the orders of your physician. Services available include Skilled Nursing, Medical Social Work, Certified Nursing Assistants, Physical Therapy, Occupational Therapy, Speech Therapy, and IV Therapy.
